Scope of this policy
This policy applies to information collected through the Quantize Health public website, business inquiry forms, and meeting-scheduling flow. It does not describe the privacy practices of independent physicians, pharmacies, healthcare providers, or a customer-facing patient portal.
Our public inquiry form is for business conversations only. Please do not include symptoms, diagnoses, treatment details, patient information, or other protected health information. This channel is not monitored for urgent or clinical needs.

Information collected with your submission
We may also record the page where your inquiry began, the referring page, campaign parameters, form progress and completion status, scheduling status, and timestamps associated with those actions. We use this limited context to understand how inquiries reach us and to keep our lead records accurate.

We do not use information submitted through this form to make clinical decisions or provide medical advice.
How we share information
We do not sell business inquiry information. We may disclose it only as reasonably needed for the purposes described in this policy, including to service providers that help us operate our website, communications, scheduling, and internal lead-management workflow.
Google services
Our inquiry workflow uses Google Workspace services, including Google Sheets, Google Calendar, and Google Apps Script, to receive and organize inquiries and coordinate meetings. If you choose to schedule a meeting, information you enter into Google’s scheduling experience may also be handled according to Google’s Privacy Policy.
Other permitted disclosures
We may disclose information when required by law, legal process, or a valid government request; when needed to prevent fraud, abuse, or security threats; or in connection with a financing, reorganization, acquisition, or sale of all or part of our business, subject to appropriate confidentiality protections.
Retention and security
We retain business inquiry information for as long as reasonably necessary to manage the inquiry, maintain business records, improve our services, resolve disputes, enforce agreements, and meet legal obligations. Retention periods may vary depending on the nature of the relationship and the information involved.
We use reasonable administrative, technical, and organizational safeguards designed to protect the information we maintain. No method of transmission or storage is completely secure, so we cannot guarantee absolute security.
Your choices and rights
Fields marked optional may be left blank. Scheduling a meeting is also optional. You may contact us to request access to, correction of, or deletion of the business inquiry information associated with you. Depending on where you live, applicable law may provide additional privacy rights.
We may need to verify your identity before completing a request. We may also retain limited information when required or permitted by law, including to maintain security, prevent fraud, or document a completed request.
